Home and Community-Based Services (HCBS)
Home and Community Based Services (HCBS) are person-centered services that allow people to live, learn, and work in their own homes and communities.
Intermediate Care Facilities (ICF)
An Intermediate Care Facility is a long-term, residential, group living healthcare model which provides individuals with 24-hour nursing services and is designed to support individuals with maintaining, learning, or regaining skills needed to live more independently. Individuals with disabilities are assisted with personal cares, activities of daily living, health monitoring, meal preparation and behavioral services, while encouraged to participate in activities to increase their independence.
Community Employment Services
Community Employment Services fosters partnerships with regional employers to identify workforce needs and create meaningful opportunities for individuals served. By aligning employer needs with the skills and aspirations of individuals, the program facilitates placements not just in jobs, but in fulfilling careers that align with their goals.
Host Homes
Host Homes are a community-based living option where individuals or families welcome a person with disabilities into their home, providing support in a family-like environment and creating a shared living arrangement.
Smart Living
Smart Living Model of Care incorporates technology and remote support technology into any of your preferred Home and Community-Based Services. Support technology ranges from smart watches to a fully remote support team at an individual's fingertips.

Spiritual Life is about harmonizing the mind, body, and spirit to empower individuals to live their best lives. It emphasizes the importance of caring for the whole person—physically, mentally, emotionally, and spiritually.
